What is the Thorncliffe Park Community Hub (TPCH)?
A Hub is a place in the heart of a neighbourhood that provides access to programs, services, and community spaces for residents in a central location. The Thorncliffe Park Community Hub is exactly that, with a mission to build healthy and connected communities in Thorncliffe Park by providing innovative integrated care, services, and support in a community-driven, inclusive space. It’s located on the 2nd floor of East York Town Centre (45 Overlea Blvd) and will house multiple partners in one space.
What programs, services, and community spaces will be provided at the TPCH?
The core provisions offered at the Hub will be for the health and well-being of the neighbourhood. Ongoing community engagement and consultation will help develop these provisions to ensure that they will work for all of the local communities. A sample of these proposed offerings are:
Primary care, diagnostic imaging and specialists
Mental health services
Health promotion, nutrition & education
Homebound seniors visiting program
Physiotherapy
Prenatal/pregnancy/postpartum care
Services for non-insured
Toronto Public Health dental
Community resource access and event space
Economic and community development
Family (child, youth, senior) services
Housing & tenancy support
Legal support & education
Newcomer/immigration support
Supports for persons with disabilities
What steps are involved to complete the TPCH?
Building the TPCH is a complex project requiring coordination from multiple stakeholders. Timelines regularly shift and are contingent on the status of any given milestone. To date, a few notable project milestones include securing the lease, piloting integrated health service delivery via Health Access Thorncliffe Park (HATP), forming an interim governance model and Steering Committee, completing base building work on the hub space, procuring professional services, and completing the Ministry of Health’s (MOH) Community of Health Capital Process to secure funding. Anticipated milestones for 2022 onwards include receiving a grant from the MOH, continued community and stakeholder engagement, beginning space mapping, and initiating the tender process for construction.
How will the TPCH be governed?
The governance structure is interim and will be reviewed by community engagement. The proposed structure consists of a TPCH advisory layer with multiple tables (residents, board chairs, primary care, to name a few), a decision-making Steering Committee, and the TPCH Management Team.
Who are the TPCH partners?
Funding partners include the City of Toronto, Ministry of Health and Long-Term Care, and United Way. Anchor partners include Michael Garron Hospital, The Neighbourhood Organization, and Flemingdon Health Centre. Engaged partners are service providers and tenants, and include Health Access Thorncliffe Park, Don Valley Community Legal Services, Midwives Clinic of East York-Don Mills, Home and Community Care, Toronto Public Health Dental, and other Primary Care providers.
