Health Access

Health Access Thorncliffe Park (HATP) provides health and wellness services and connects people to other resources. HATP is co-led by Flemingdon Health Center and TNO (The Neighbourhood Organization).

About

At HATP, we care about you as a whole person. Our team talks to you about all your needs. We explore your physical health, your emotional and social well-being and we connect you to any services or supports you may need. Our services are FREE and open to all residents living in the M4H postal code. Our services can be accessed without a health card and without a medical referral. Language and interpretation services are available as needed.

Health Access Thorncliffe Park (HATP) is an initiative co-led by FHC and TNO (The Neighbourhood Organization) to provide comprehensive primary health care services to residents of Thorncliffe Park.

Primary Health Care 

Our team of health care professionals work with you to provide ongoing, person-centered comprehensive medical care that looks at all your needs and aims to keep you healthy.

Pediatric Clinic

The school-based pediatric clinic is uniquely focused on the learning, growth, developmental and behavioural concerns of children.

Foot Care

Basic foot-care services for seniors and for people with chronic health conditions.

Nutrition

Support for adults, families and children in making healthy food choices and managing concerns like picky eating and high cholesterol.

Social Work

Individual, family and group mental health counselling, advocacy, case management and support to people of all ages.

Health Education and Promotion

Programs and workshops that address the specific needs of the neighbourhood and enhance the well-being of the community.

Community Resources and Referrals 

Making seamless connections and referrals to services including income support, settlement, housing, employment and language instruction.

Family Support

Holistic care to support the health of the whole family, from healthy child and youth development to supports for seniors.
