The Thorncliffe Park Community Hub is a welcoming, accessible, and inclusive multi-service space designed to serve the vibrant communities of Thorncliffe Park, Flemingdon Park, and nearby neighborhoods. The Hub is your destination for a wide range of health and social services, programs, and community gathering spaces that inspire connection, well-being, and empowerment.

With a mission to foster meaningful connections and provide vital resources, the Hub is here to support residents in living healthy, fulfilling lives within a thriving community.

Why Hub?

In 2014, local organizations, residents, and stakeholders undertook a Health Care Gap Analysis of Thorncliffe Park. It quickly became evident that, given the population’s size, complexity and diversity, there are insufficient local health services. Many living in Thorncliffe Park are in poorer health than other Toronto residents. Social and economic conditions in the community such as high un(der)employment, isolation, language and cultural barriers, overcrowded living conditions and the stresses of resettlement contribute to the neighbourhood’s health challenges. The Gap Analysis concluded that there is a need for comprehensive, coordinated primary health care that is responsive to the populations’ unique health and social needs and integrated in the neighbourhood.

Governance

The Thorncliffe Park Community Hub (TPCH) operates under a collaborative governance model led by a Steering Committee

Which guides strategy and supervises Hub activities. This committee includes representatives from all partner organizations and community members, promoting shared responsibility and equal input from all involved.

The Hub management team oversees partnership coordination and daily operations to ensure smooth functioning and alignment with the Hub’s mission.
